We know that when you are dealing with a serious illness, home is the place you feel most comfortable. Our palliative teams provide care in your place of residence, wherever that may be, and that care includes comprehensive medical assessments, plus ongoing follow-ups and management.

Our palliative teams recognize that all adult persons have a fundamental right to make decisions relating to their own medical treatment. This includes the right to accept or refuse medical care, and we will work closely with you to document your wishes. Our team will work with you and your family to establish personal goals of care and assist you in achieving those goals—knowing that goals may change over time.

Your comfort is of utmost importance. Our purpose is to partner with you to understand your goals so we can help you remain comfortable. Your nurse or nurse practitioner will ask you and your caregiver about pain and other symptoms, and they will use this information to develop a symptom management plan with you and your physician.

Our palliative care team will work closely with you, your family, and your physician, and any other necessary healthcare providers to develop a plan of care that meets your care goals and helps ensure that you are in the right care setting to meet the needs of your serious illness.
